Job summary

Enhabit Home Health & Hospice in Helena, MT, seeks a Care Transitions Coordinator (Clinical Liaison) to guide patients through post-acute care and improve outcomes while boosting referral source satisfaction.

You will develop patient-centered transition plans using evidence-based guidelines, represent Enhabit in strategic health-system partnerships, and ensure compliance with Medicare certified home health and hospice regulations.

Qualifications

• Must understand federal, state, and local laws governing Medicare home health and hospice operations.

Overview

As a Care Transitions Coordinator (Clinical Liaison), you will assist patients with the process of navigating post-acute care. Your goal is to create a positive impact on patient outcomes while increasing referral source satisfaction with Enhabit's services.

• Integrate evidence-based clinical guidelines, preventative guidelines, protocols, and other metrics in the development of transition plans that are patient-centered and promote quality and efficiency in the delivery of post-acute care.
• Represent the area home health branches in strategic relationships with health systems, hospitals, inpatient facilities, physicians and physician groups, and executive level opportunities.

Responsibilities

• Integrate evidence-based clinical guidelines, preventative guidelines, protocols, and other metrics in the development of transition plans that are patient-centered and promote quality and efficiency in the delivery of post-acute care.
• Represent the area home health branches in strategic relationships with health systems, hospitals, inpatient facilities, physicians and physician groups, and executive level opportunities.

Qualifications

• Must be a graduate of an approved school of nursing or therapy.
• Must be licensed in the state where they currently practice.
• Must have two (2) years of demonstrated field experience.
• Must have demonstrated experience and understand federal, state, and local laws and regulatory guidelines governing the operations of Medicare certified home health and hospice.
• Must have basic demonstrated technology skills, including operation of a mobile device.

Education And Experience, Preferred

• A Registered Nurse RN or Physical Therapist PT is required.
• Three (3) years of field experience is preferred.
• Previous experience in home health or healthcare sales is preferred.

Requirements

• Must possess a valid state driver license
• Must maintain automobile liability insurance as required by law
• Must maintain dependable transportation in good working condition
• Must be able to safely drive an automobile in all types of weather conditions
