Job summary

BaneCare Management is seeking a dynamic Clinical Liaison to serve as the primary contact between referral sources, healthcare providers, patients, and our organization in the Boston area. The role focuses on building referrals, educating clinicians on services, and ensuring smooth transitions of care across settings.

Ideal candidates are licensed clinicians with strong communication skills and knowledge of acute, post-acute, rehab, hospice, home health, or long-term care settings.

Qualifications

• Current RN or LPN preferred; other education and experience considered.
• Minimum of 1 year of experience in healthcare marketing, business development, admissions, case management, or clinical liaison roles.
• Strong understanding of healthcare reimbursement, referral processes, and care transitions.
• Valid driver's license and ability to travel throughout the Greater Boston area.

Responsibilities

• Build and maintain relationships with hospitals, physicians, case managers, discharge planners, SNFs, and other referral sources.
• Conduct patient assessments to determine eligibility and appropriateness for services.
• Coordinate admissions and facilitate smooth transitions of care.
• Educate referral sources, patients, and families regarding available programs and services.
• Collaborate with clinical and operational teams to ensure timely and accurate admissions.
• Monitor referral activity and maintain accurate documentation in CRM and EMR systems.
• Attend community events, networking opportunities, and professional meetings to promote organizational services.
• Analyze market trends and identify opportunities for growth within the assigned territory.
• Ensure compliance with all federal, state, and organizational policies and regulations.
