Spaulding Rehabilitation Hospital in Boston is seeking a Community Outreach Manager to advance partnerships with local providers and community organizations, promoting the hospital's services and facilitating patient transitions.
The role requires MA-licensed clinicians preferred, 2–3 years outreach, 1–2 years managed care, with travel in the community; 40-hour week on a hybrid schedule with some weekend/holiday rotation.

This is a full-time position working a hybrid schedule of 4 days in person and 1 day remote. This position also rotates weekends and holidays.
Responsible for establishing and maintaining strong connections with community partners, healthcare providers, and relevant organizations by promoting the hospital's services, building relationships, and facilitating seamless patient transitions between the community and hospital settings.
