Job summary

City of Hope is seeking a Clinical Informatics Pharmacist to bridge clinical care and IT, focusing on medication management within Epic and related clinical applications. The role involves collaboration with physicians, nurses, pharmacists and IT, and ensuring evidence-based practice in system design.

Ideal candidates have PharmD, active license, and 5+ years of pharmacist experience or 3+ years with 2 years IT care. Travel to clinics may be required.

Qualifications

• Doctor of Pharmacy (Pharm D) degree is required.
• Active license with Board of Pharmacy is required.
• 5+ years in a pharmacist role or 3 years plus 2 years in healthcare IT experience.
• Experience with clinical information systems and medication management.
• Ability to collaborate with physicians, nurses, pharmacists and IT professionals.

Responsibilities

• Provide leadership on the design, development, and implementation of clinical information systems for medication management.
• Lead multidisciplinary teams to design future state medication management.
• Facilitate reviews of order sets and treatment plans per COH policy.
• Coordinate teams and direct members for project planning and execution.
• Assist in designing user-friendly interfaces that present content logically for clinical use.
• Develop standardized content with the Pharmacy system in mind.
• Coordinate Clinical Pathway activities.
• Assess clinician enhancement requests and advance them through governance processes.
• Assist IT to troubleshoot and resolve issues.
• Apply project management methodologies when appropriate.
• Ensure design aligns with evidence-based medicine and guidelines.
• Maintain content updates per evidence and clinical requirements.
• Promote standardization across COH system.
• Understand requirements from pharmacy, clinical, quality, regulatory, and operations.
