Job summary

Hackensack Meridian Health is seeking a Pharmacist to provide comprehensive pharmaceutical care through integrated drug distribution and clinical services, ensuring safe and effective medication use.

The Pharmacist dispenses and monitors therapy, adheres to USP standards, oversees pharmacy staff, and collaborates with providers to optimize patient outcomes in inpatient settings. A BS in Pharmacy and NJ Pharmacist License are required.

Qualifications

• Graduate of an ACPE accredited school of pharmacy, or equivalent, with a minimum of a BS degree in Pharmacy.
• Education, Knowledge, Skills and Abilities Preferred: 1-2 years of hospital pharmacy experience.
• Knowledge of principles of USP 795, 797, and 800.
• EPIC experience.

Responsibilities

• Complies with and enforces all state and federal regulations and policies relating to pharmaceutical care and medication management.
• Review and verification of all medications prior to dispensing from the pharmacy department; communication with providers when necessary.
• Oversee dispensing, chain of custody, and accountability of controlled substances.
• Adheres to USP 795, 797, and 800 standards in compounding and hazardous drugs.
• Oversee ancillary personnel in pharmacy operations.
• Provide pharmacological information to healthcare professionals; participate in clinical initiatives and patient counseling.
