Qualifications

Responsibilities

• Verify and review medication orders for dose, duration, and indication.
• Monitor therapy for drug interactions and safety; ensure formulary compliance.
• Provide counseling to patients on medication use and discharge instructions.
• Participate in medication reconciliation and pharmacist rounds as needed.
• Support antimicrobial stewardship and pharmacokinetic considerations.
