Job summary

Qualifications

• CA Pharmacist License (CA State Board) is required.
• 3 years hospital pharmacy experience; or 1 year PGY1 residency.
• ACLS certification required; BLS for Healthcare Providers required.

Responsibilities

• Verify and review medication orders for dose, duration, and indication.
• Monitor therapy and adjust regimens per approved protocols.
• Counsel patients on medication use and discharge education.
• Participate in rounds and collaborate with the interdisciplinary team.
• Oversee pharmacy operations and ensure regulatory compliance.
