Job summary

CentraCare Health in St. Cloud is seeking a Heart Failure Coordinator to lead the heart failure program, ensuring delivery of high-quality, evidence-based patient care across the continuum.

You will coordinate multidisciplinary teams, support patient and family education, and monitor clinical outcomes to reduce readmissions. This full-time role requires a nursing background, a BSN or MSN preferred, and at least five years of ICU, cardiology, or ED experience.

Qualifications

• Graduate from a nursing program.
• BSN or MSN preferred.
• BSN or higher required within 36 months of being hired.
• 5 years minimum nursing practice in ICU, cardiology or ED.
• RN license in Minnesota and BLS in good standing.

Responsibilities

• Provide leadership and oversight of the heart failure program.
• Coordinate multidisciplinary care and patient/family education.
• Monitor quality metrics and regulatory readiness.
• Lead initiatives to improve outcomes and reduce readmissions.
• Serve as a clinical resource and collaborate with care teams.
