Acentra Health is seeking a Clinical Evaluator in Omaha, Nebraska, to conduct in-person assessments for individuals with mental health needs. Make a meaningful impact in your community-join Acentra Health as a Clinical Evaluator (PASRR) in Omaha, Nebraska. In this hybrid role, you will conduct in‑person preadmission evaluations and assessments throughout Omaha and its surrounding areas.
As a Clinical Evaluator, you will conduct face‑to‑face PASRR Level II assessments to determine appropriate placements and next levels of care for individuals with complex needs. Your primary focus will be completing clinical evaluations for individuals with Serious Mental Illness (SMI), Intellectual Disabilities (ID), or Related Conditions (RC) across nursing homes, hospitals, and community‑based settings. These evaluations are crucial in determining the next level of care, improving patient well‑being, and supporting better health outcomes.
This PRN hybrid role offers flexibility and purpose, combining remote work from your home office with local travel as needed. Travel expenses—including mileage and parking—are covered and reimbursed by Acentra Health. We will provide all necessary technology equipment for your home office, including a laptop for use in the field.

For non‑clinical roles, or when not required by the contract specifically, the Company acknowledges that practical, hands‑on experience can provide skills and competencies equivalent to formal education. As such, in cases where a Bachelor's degree may be required, the Company will accept a minimum of six (6) years of directly relevant professional experience in lieu of a degree. In instances where the candidate has an Associate's degree, the Company will accept a minimum of three (3) years of directly relevant professional experience in lieu of the Bachelor's degree.
The compensation for this role is $36.00 to $40.00 per hour.
