Acentra Health is seeking a Clinical Evaluator - PRN to join our growing team. You will perform PASRR Level II assessments to determine the appropriate placement for individuals with complex needs across nursing homes, hospitals, and community settings.
In this hybrid role, you’ll combine in-person evaluations with remote work from your home office, traveling to Topeka or Wichita areas. Travel expenses are covered, and you’ll have access to our technology setup.

Acentra Health is looking for a Clinical Evaluator - PRN to join our growing team
As a Clinical Evaluator, you will conduct face-to-face PASRR Level II assessments to determine appropriate placements and next levels of care for individuals with complex needs. Your primary focus will be completing clinical evaluations for individuals with Serious Mental Illness (SMI), Intellectual Disabilities (ID), or Related Conditions (RC) across nursing homes, hospitals, and community-based settings. These evaluations are crucial in determining the next level of care, improving patient well-being, and supporting better health outcomes.
The list of accountabilities is not intended to be all-inclusive and may be expanded to include other education- and experience-related duties that management may deem necessary from time to time.

For non-clinical roles, or when not required by the contract specifically, the Company acknowledges that practical, hands-on experience can provide skills and competencies equivalent to formal education. As such, in cases where a Bachelor's degree may be required, the Company will accept a minimum of six (6) years of directly relevant professional experience in lieu of a degree. In instances where the candidate has an Associate's degree, the Company will accept a minimum of three (3) years of directly relevant professional experience in lieu of the Bachelor's degree.
The compensation for this role is $36.00 to $40.00 per hour
