Job summary

Moffitt Cancer Center is seeking a Clinical Documentation Specialist II to improve the quality and completeness of provider-based clinical documentation in the medical record. You will work directly with treating providers to ensure documentation accurately reflects illness severity, mortality risk, and care complexity.

You will review records, apply AHIMA/ACDIS standards, and collaborate with physicians and nurses to educate and refine documentation practices, using tools like AI and EHR

Qualifications

• Associate degree in Nursing, HIM, or healthcare field required.
• At least four years acute care clinical documentation experience.
• Experience with Medicare/Medicaid and payer regulations, coding guidelines.
• Familiarity with ICD-10-CM/PCS coding.
• Ability to perform independent queries.

Responsibilities

• Facilitate improvement in provider-based clinical documentation quality.
• Work with providers to ensure documentation reflects severity and complexity.
• Review records and apply evolving AHIMA/ACDIS standards.
• Collaborate with physicians and nursing to educate on documentation.
• Conduct independent research for compliant provider queries.
