The actual pay rate and pay grade for this position will be dependent on a variety of factors, including an applicant’s years of experience, unique skills and abilities, education, alignment with similar internal candidates, marketplace factors, other requirements for the position, and employer business practices. **What You Will Do** Manages the nutritional care of infant, adolescent, adult and geriatric patients through assessment of nutritional status, formulation, implementation and monitoring of care plans following appropriate policy/procedure. Utilizes unique knowledge of nutrition, interprets and communicates nutrition care principles to clients and patients. Collaborates with other professionals and personnel in the integration and delivery of care. **Required Qualifications*** AHA Basic Life Support for Healthcare Professional (AHA BLS Healthcare) - American Heart Association -REQUIRED**Other Qualification Requirements*** Valid CA drivers’ license required, Auto Insurance required (for Home Care position only). Registered Dietitian or Registered Dietitian Nutritionist, required.
