Job summary

Henry Ford Health is seeking an expert Clinical Nutritional Consultant to deliver end-to-end, personalized nutrition care for patients and families. You will assess, diagnose, implement care plans, counsel, and evaluate outcomes while coordinating with the healthcare team.

The role includes education program development, mentorship of dietetic interns, and collaboration across departments to promote wellness, with emphasis on discharge planning and community outreach.

Qualifications

• Bachelor’s Degree in Dietetics or related field with ACEND-accredited DPD and supervised practice; RD credential active if obtained before 2024.
• Master’s Degree in Dietetics or related field with ACEND-accredited DPD and supervised practice; Advanced Practice Certification preferred.

Responsibilities

• Conduct comprehensive patient assessments including data interpretation.
• Diagnose nutrition-related issues using standardized nutrition diagnoses.
• Design and implement individualized nutrition therapy and care plans.
• Monitor progress and adjust plans based on outcomes and labs.
• Educate patients and families on prescribed diet plans and nutrition topics.
• Develop and update educational materials for patients and community.
• Collaborate with medical staff and other departments to ensure optimal care.
• Precept and mentor dietetic interns and supervise students.
• Maintain accurate clinical documentation in EMR following policies.
