Qualifications

Responsibilities

• Assess patients' nutritional needs and develop dietary plans.
• Counsel patients on post-discharge diets and healthy eating.
• Coordinate with physicians and care team on nutrition care.
• Document nutrition care in medical records per standards.
• Evaluate referral orders and assess adequacy of intake methods.
• Conduct calorie counts and adjust menus for needs.
• Review menus for nutritional adequacy and sign off.
