Montefiore Medical Center in New York is seeking a dedicated dietitian to provide in-depth nutritional assessment and follow-up for patients at high nutritional risk. You will develop care plans, including discharge diet therapy instructions, and participate as a core team member in interdisciplinary patient care programs.

You will assess level of care, document care plans, and educate patients and families.

Qualifications

• BS in Nutrition/Dietetics required.
• MA or advanced degree preferred.
• Registered Dietitian/Nutritionist credentials required.

Responsabilités

• Assess nutritional status and develop care plans.
• Discharge diet therapy instructions and follow-up.
• Collaborate with interdisciplinary care teams.
• Document assessments, plans, and progress notes.
