Job description

Overview

Provides clinical nutrition services to inpatients and outpatients. Assesses patient status and develops an individualized nutrition care plan in accordance with the Nutrition Care Process and regulatory standards. Participates in the interdisciplinary team and conducts training/education programs as required.

Responsibilities

• Provides all aspects of Medical Nutrition Therapy to patients using evidence-based research in accordance with the Nutrition Care Process (NCP) and Clinical Nutrition Standards of Practice.
• Assesses the nutrition status of patients relative to their diagnosis and medical condition using medical records, anthropometric data, visual assessment, and other subjective data.
• Develops and implements a plan of care to meet identified patient problems or needs, writes measurable expected outcomes, and plans according to patient progress.
• Documents evaluation of patient care outcomes through direct interview, nutrient intake calculations, and monitoring tolerance of nutrition therapy interventions.
• Demonstrates knowledge, technical, interpersonal, and critical thinking skills to provide care, maintain clinical competency, and meet age-specific patient requirements/standards.
• Provides follow up with patients and family members regarding nutrition goals and individualized care plans.
• Assists in implementing standards for patient-centered care, instruction, and methods of evaluating care and instruction.
• Works within patient care policies and procedures, considering risk management, infection control, and patient safety practices.
• Participates in interdisciplinary team conferences to ensure optimal patient care.
• Assesses learning needs of patients and families and provides education as indicated.
• Collaborates with other healthcare team members in planning, implementing, and evaluating nutrition care plans.
• Documents all pertinent information in the medical record clearly and within appropriate time frames.
• Acts as a liaison between the patient, nursing, and medical staff to answer questions and resolve concerns relating to clinical nutritional services.
• Maintains patient medical records; performs charting duties; uses electronic health records to enter and retrieve data.
• Maintains documentation to comply with third-party billing standards.
• Communicates with patient/family/clinic/hospital staff regarding all aspects of patient needs in a courteous manner.
• Complies with organizational standards and promotes patient/customer satisfaction.
• Maintains professional credentials and continuing education to stay current with clinical principles and techniques. Attends mandatory programs and ongoing staff development.
• Maintains a clean, orderly, safe environment for patients and personnel and adheres to policies, compliance/quality improvement, patient safety, and infection control standards.

Qualifications

• Bachelor’s Degree in foods and nutrition from an accredited school with a major in nutrition and completion of a dietetic internship or coordinated program approved by the Academy of Nutrition and Dietetics. Required.
• As of January 2024, a Master’s Degree is required for all newly credentialed practitioners. Required.
• Qualified to work with neonate, infant, adolescent, adult, and/or geriatric populations as assigned.
• REG DIET - Dietitian – Registered with the Commission on Dietetic Registration (CDR) of the Academy of Nutrition and Dietetics. Upon hire.
• Licensed by the State of Tennessee Board of Dietitian/Nutritionist Examiners. For new residents: must obtain a state license within 180 days.
• Minimum 2 years of experience with a bachelor’s degree in varied clinical settings (e.g., ICU, step-down, cardiology, med/surgical units, patient education for individuals and groups). Required.
• No prior experience required for newly credentialed graduates with a master’s degree.
• Minimum 4 years of experience in varied clinical settings is preferred.
