Job Description
The Clinical Dietitian Nutritionist 2 is responsible for providing Medical Nutrition therapy to patients in both the inpatient and outpatient setting, including nutrition assessment, diagnosis, and intervention in patient care management. Assesses nutritional needs and identifies nutrient requirements based on age, disease, and individual needs for all nutritionally at-risk patients. Develops, implements, and monitors nutrition care plans. Coordinates nutritional aspects of patient care with physicians, providers, and other healthcare personnel. Serves as a resource for Clinical Dietitian Nutritionist 1 and other health care professionals. Participates in medical rounds, discharge planning, and patient care conferences as appropriate for the clinical assignment. Acts as a preceptor and assists in coordinating daily schedules and assignments for nutrition interns.
Responsibilities
- Patient Care
- Provides nutrition consultation and education for patients and their families at nutrition risk, identified with nutrition risk, or referred by healthcare providers for nutrition-related problems, including chronic disease management and nutrition support.
- Acts as a Specialist Nutrition for the Clinical Nutrition team.
- Applies the Nutrition Care Process, including assessment, diagnosis, and evidence-based intervention.
- Performs nutrition-focused physical exams (NFPE) during assessments to determine malnutrition risk or severity when indicated.
- Evaluates patient progress and adjusts care plans accordingly.
- Communicates the nutrition care plan effectively with the care team.
- Follows department policies for nutrition screening and assessment.
- Provides inpatient and outpatient interventions and education, developing individualized nutrition care plans based on thorough assessments.
- Uses medical nutritional therapy knowledge and considers age-specific nutritional needs.
- Addresses questions and concerns of patients, families, and healthcare professionals using evidence-based practices.
- Utilizes monitoring and evaluation techniques considering cultural, religious, and ethnic factors.
- Documents all patient encounters appropriately, including PES format for diagnoses.
- Manages formula selections to meet age, disease-specific, and financial needs.
- Ensures proper use of protected health information (PHI) and safety practices.
- Assists in discharge planning with the multidisciplinary team, ensuring access to resources like WIC and DME supplies.
- Education/Professional Development
- Serves as an educator and resource on nutrition issues, supporting staff and community education.
- Functions as a clinical preceptor and participates in teaching roles for other health professionals, patients, and the community.
- Supports professional growth through activities, workshops, and seminars.
- Applies standards of practice and performance to ensure competence and program quality.
- Performance Improvement
- Provides suggestions for program development and evaluates existing programs for quality and efficiency improvements.
- Adheres to policies, safety, quality, and infection control standards.
- Participates in meetings and quality improvement activities.
- Uses evidence-based practices and stays current with clinical developments.
- Supports compliance with regulatory standards and monitors documentation methods.
- Performs other duties as assigned.
Qualifications
- Experience: Minimum of three years practicing as a clinical Registered Dietitian Nutritionist.
- Education: Bachelor's Degree, completion of an Accredited Dietetics Internship, and preferably a Master’s degree in a nutrition-related field. Current Board Certification is preferred.
- Licensure: Louisiana Licensed Dietitian Nutritionist and Registered Dietitian Nutritionist with the Commission on Dietetic Registration.
