Job summary

Sentara Health Plans is seeking a Clinical Dietitian for outpatient rehab therapy centers in Gloucester, VA and Coliseum (Hampton). This full-time hybrid role offers remote flexibility with on-site days split between locations and a day shift schedule.

You will assess and manage nutrition for acute and chronic diseases, provide counseling, develop care plans, and participate in community outreach. RD credentialing and BLS are required within specified timelines.

Qualifications

• BLS is required to be obtained within 90 days of hire.
• RD must be obtained within 6 months of hire. Verification statements required for those awaiting RD credentialing.
• If Bachelor's Degree is in major other than Dietetics, must complete an ACEND accredited DP in Dietetics.
• NBCOT certification is required within 6 months of eligibility.

Responsibilities

• Develop and implement a comprehensive care plan based on patient/caregiver needs.
• Perform nutrition assessments of diverse disease states and provide counseling and culinary nutrition.
• Represent Sentara at health fairs, media outreach, and community presentations.
• Collaborate with providers and healthcare team for referrals, data, and rounds.

Overview

Sentara is hiring a Clinical Dietitian for the following outpatient rehab therapy centers: Sentara Therapy Center Gloucester, Sentara Therapy Center Coliseum (located in Hampton). This is a hybrid role that offers remote flexibility with on-site days divided between the Gloucester and Coliseum locations. This is a Full-Time, day shift opportunity with great benefits.

Responsibilities

• Responsible for the assessment and nutritional management of acute and chronic disease in the outpatient setting.
• May include individual and group counseling, culinary nutrition, community outreach, public education and scheduling.
• Develops and/or implements a comprehensive care plan based on assessment and evaluation of patient/caregiver needs.
• Performs nutrition assessment of a wide variety of acute and chronic disease states, individual and group counseling, culinary nutrition.
• Serves as a Sentara representative for health fairs, media outreach, and community presentations.
• Communicates and collaborates with providers and other members of the healthcare team for referrals, clinical diagnostic data, weekly rounds and any other patient care meetings as needed.
