Qualifications

• RN licensed with at least 1 year of long-term care or acute care experience.
• Minimum of 1 year of formal leadership experience in health care.
• Rehabilitation experience is highly desirable.
• Current CPR certification is required.
• Excellent communication skills are essential.
• Willingness to support and lead others to uphold St. John’s Brand Characteristics.
• Compliance with NYS Department of Health immunization requirements.

Responsibilities

• Coordinate clinical services for each resident.
• Develop individualized care plans.
• Generate nursing staff assignments with thorough communication.
• Provide emotional, psychological, social and spiritual support to residents and families.
• Assist floor nurses with bedside care and conduct periodic rounds.

Position Summary:

The Clinical Coordinator is responsible for supporting and leading dedicated nursing staff . The Clinical Coordinator ensures staff assignments provide care that enhances residents' ; collaborates with medical providers; assists with staff development; and oversees clinical operations leading to positive rehabilitation outcomes.

The schedule for this position will be Monday-Friday on days with every other weekend required.

Pay is commensurate with years of clinical and leadership experience. Generous extra pay for hours worked past 3:00 pm and on weekends.

Qualifications / Education:

• Current RN a minimum ofone (1) year of long term care or acute care experience required
• Minimum of one (1) year formal leadership experience in health care required
• Previous Rehabilitation experience highly desirable
• Current CPR certified required
• Excellent communication skills
• Willingness to support and lead others to living St. John’s Brand Characteristics
• Compliance with NYS Department of Health immunization requirements.

Physical Requirements:

• Sits, stands, walks most of the day. Frequent moving and lifting of residents. Moderate to heavy lifting (up to 50 lbs).

• May be exposed to infection, odors, psycho-behavioral situations.

• Responsible for coordination of clinical services for each resident
