The Clinical Coordinator reflects the mission, vision, and values of NM, adheres to the organization’s Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.
The Readmissions coordinator works with a multidisciplinary team to provide the best care for patients in the right setting while helping prevent readmissions. The Navigator will utilize hospital resources efficiently to expedite testing that must be done urgently and set up any remaining care with outpatient continuity providers. Focus will be on patients who are high utilizers of the hospital and working to decrease low value care while supporting primary teams to provide consistent care across different hospitalizations. In depth assessments of patients will be utilized to better understands barriers to outpatient management, health literacy gaps, SDOH etc. The Navigator will work closely with emergency room providers, hospitalists, APPs, SWs and other staff in developing care plans.
