  • Active New York State Respiratory Therapist License and Registered Respiratory Therapist (RRT) credential issued by the NBRC.
  • Clinical respiratory therapy experience, with strong preference for home mechanical ventilation, multidisciplinary ALS or neuromuscular clinics, pulmonary specialty care, or sleep medicine.
  • Demonstrated expertise with current home ventilators (e.g., Philips Trilogy, ResMed Astral), respiratory assist devices (e.g., ResMed AirCurve), airway clearance technologies, and cloud-based management platforms.

Responsibilities

  • Lead in-clinic assessment, initiation, and optimization of NIV and MPV; customize therapy settings to clinical goals.
  • Educate patients and caregivers on mechanical insufflation-exsufflation, cough assist devices, and related technologies.
  • Review ventilator data, capnography, and overnight oximetry to assess patient-ventilator synchrony and adherence.
  • Educate patients and families on device operation, mask fitting, cleaning, and emergency troubleshooting.
  • Perform bedside spirometry, peak cough flow, ABG sampling; ensure infection prevention and regulatory standards; document in EHR.

Position Summary

Columbia University Irving Medical Center (CUIMC) is seeking a highly skilled and visionary Registered Respiratory Therapist (RRT) to help build and expand the newly established Claire Tow Home Assisted Ventilation Program within the Division of Pulmonary, Allergy, and Critical Care Medicine.

This innovative program provides comprehensive, multidisciplinary care to patients with complex respiratory conditions, including neuromuscular diseases, severe chronic obstructive pulmonary disease (COPD), and chronic respiratory failure. We are developing a forward-thinking clinical model that leverages telemonitoring and cloud-based ventilator data to proactively manage respiratory health and optimize patient outcomes. As the program's inaugural Lead Outpatient Respiratory Therapist, you will play a critical role in designing clinical workflows, establishing ventilator titration protocols, and delivering highly personalized patient care in support of the program's clinical, educational, and academic mission.

Responsibilities
  • Clinical Setup and Titration: Lead the in-clinic assessment, initiation, and optimization of noninvasive ventilation (NIV) and mouthpiece ventilation (MPV), as appropriate. Individualize therapy by adjusting ventilator modes, inspiratory times, pressures, and other settings to achieve established clinical goals.
  • Airway Clearance Management: Educate patients and caregivers on the use of mechanical insufflation-exsufflation (cough assist) devices, high-frequency chest wall oscillation systems, lung volume recruitment techniques, and targeted secretion management strategies.
  • Telemonitoring and Data Analytics: Perform ongoing review and interpretation of ventilator downloads, capnography data, and overnight oximetry studies to assess patient-ventilator synchrony, adherence, and evidence of hypoventilation.
  • Patient and Caregiver Education: Serve as the primary educator for patients and families regarding device operation, mask fitting, equipment maintenance, cleaning procedures, and emergency troubleshooting.
  • Clinical Testing and Program Support: Conduct bedside spirometry, peak cough flow assessments, and arterial blood gas sampling; calibrate respiratory equipment; ensure compliance with infection prevention, patient safety, and regulatory standards; support efficient clinic operations and patient flow; participate in quality improvement initiatives; and document all clinical activities within the electronic health record.
Minimum Qualifications
  • Bachelor's degree or equivalent in education and experience, plus four years related experience.
  • Active New York State Respiratory Therapist License and Registered Respiratory Therapist (RRT) credential issued by the National Board for Respiratory Care (NBRC).
  • Clinical respiratory therapy experience, with strong preference for candidates who have specialized expertise in home mechanical ventilation, multidisciplinary ALS or neuromuscular disease clinics, pulmonary specialty care, or sleep medicine.
  • Demonstrated expertise with current home ventilators (e.g., Philips Trilogy, ResMed Astral), respiratory assist devices (e.g., ResMed AirCurve), airway clearance technologies, and associated cloud-based clinical management platforms.
  • Highly autonomous, organized, and detail-oriented, with the ability to thrive in a dynamic, patient-centered environment.
  • Excellent interpersonal, communication, and patient education skills.
  • Demonstrated commitment to compassionate care and continuous quality improvement.
Preferred Qualifications
  • Experience coordinating care for medically complex patients across multidisciplinary care settings.
Other Requirements
  • Medical clearance and participation in the medical surveillance program.
  • Direct patient contact and/or interaction with human research subjects.
  • Potential exposure to bloodborne pathogens.
  • Successful completion of all required compliance and systems training.
