Responsibilities

• Coordinate care for complex and high-risk pediatric and adolescent patients with genetic skin disorders and inflammatory and autoimmune conditions.
• Collaborate with pediatric dermatology physicians to develop knowledge of disease pathogenesis, prognosis, and treatment plans.
• Assess learning needs of families and implement education plans with the care team.
• Triage messages from patients/families with the dermatology team.
• Coordinate outpatient pediatric dermatology appointments with inpatient team.
• Meet with inpatient care teams and families to develop follow-up plans and provide education.
• Communicate with patients via telephone and secure messaging to assist with coordination.
• Coordinate with home care agencies, school nurses, and primary care providers.
• Provide wound care education and assist with dermatology procedures.

Our patients have acute care needs andutilizemany services across our ambulatory and inpatient sites.This position will supportthe provision of high-quality, patient centered care fortheDepartmentof Dermatologyby providing clinical support and care coordination to ourDivisionofPediatricand AdolescentDermatology.

Responsibilities

• Coordinatecareforcomplex and high risk pediatricandadolescent patientswith genetic disordersassociated with skin disease(ichthyosis, epidermolysis bullosatuberous sclerosis, neurofibromatosis),inflammatoryand autoimmunedisorders(atopic dermatitis, psoriasis, rheumatologic disorders), cancer related skin disease andvascularanomalies (hemangiomas, vascular malformation syndromes).
• Work with the pediatric dermatologyphysicians todevelop knowledge ofdisease pathogenesis,prognosis,and treatment plansfor various pediatric dermatologic disorders.
• Assesslearning needs of the family andwork with pediatric dermatologyphysicians todevelop and implement education plan.
• Triage messagesfrom patients/families in conjunction withpediatric dermatology team(Dermatologyattending,nurse practitioner, medical assistantandfellow.)
• Coordinate outpatient pediatric dermatology appointments in collaboration with inpatient team
• Meet with inpatient care team and/or families when needed to develop follow-up plan of care, provide in-service and/or consultation to provider groups.
• Communicate with patients via telephone and secure messaging through medical record/patient portal to assist with care coordination.
• Coordinatewith home care agencies,school nursesand primary care providers.
• Patient and family education and instruction regarding plan of care and at-home self-care.
• Coordination of care across multiple specialties acrossColumbia University Irving Medical Center (CUIMC)andNew York Presbyterian- Morgan Stanley Children’s Hospital (NYP-MSCH).
• Organize/facilitatetwice monthlyMultidisciplinaryVascular Anomalies Clinic(VAC)sessionsworking alongsideaninterdisciplinary team that includespediatricspecialistsfrom multiple disciplines includingDermatology, Hematology, Genetics, lnterventional Radiology,Otolaryngology,Plastic Surgery, General Surgery, and Radiology. Review the charts of these complex patients prior to their VAC appointments, aggregate their data, ensure they have gone for the appropriaterecommendedscreening studies in advanceand after appointments.
• Partner with pediatric dermatology leadership to support other pediatric interdisciplinary initiativesincluding (Epidermolysis Bullosa Interdisciplinary sessions)
• Provide wound care education,assistwith pediatric dermatologyprocedures (e.g.,lasertreatmentsandfirst assist withexcisions).
• Provide assistancewith procedures requiring medication applications,teachingand monitoring.
• Lead and participate inquality improvement and patient experience initiativesimprovements for Dermatology.
• Perform other related duties and responsibilities as assigned/requested.

Minimum Qualifications

• Bachelor’s degree in Nursing or equivalent in education and experience, plus 4 years of related experience.
• BLS certified.
• Current New York State RN license.

Preferred Qualifications

• Experience with Pediatricand/orDermatologypatients.
• Excellent communication skills.
• Bilingual (Spanish and English).

• Assess patients using all available physical and diagnostic data, incorporating medical history, and evaluate patient status and progress with the current plan of care.
• Provide coverage and assess patients during and after selected therapies; perform CPR.
• Attend staff meetings, patient support meetings, and continuing education activities pertaining to the specialty as appropriate.
• Participate in all quality improvement/process improvement initiatives.
• Conduct comprehensive patient assessments, obtain accurate medical histories through effective interviewing techniques, ensure thorough and timely documentation in the medical record, and provide patient and family education regarding disease state, treatment plans, medications, side effects, and follow-up care.
• Perform phone triage as appropriate and provide follow-up guidance and direction to patients based on the situation.
• Participate in sign-out activities and patient follow-up.
• Ensure compliance with applicable regulatory agency standards (e.g., OSHA, JCAHO, DOH, HIPAA, billing compliance, etc.).
• Perform diagnostic testing and analysis of results.
• Order appropriate laboratory supplies, follow up on results, answer pre- and post-operative questions, gather data, and act as a liaison.
• Provide cross-coverage across clinical areas and perform other duties as assigned to support patient care, clinic operations, and departmental needs.
