Job summary

Holston Medical Group in Kingsport, TN is seeking a Clinical Chart Review professional to coordinate maintenance health care needs designed to optimize care for conditions such as diabetes, hypertension and hyperlipidemia. You will initiate care and orders for assigned patients, ensuring timely communication with providers to deliver high-quality, patient-centered care.

Minimum requirements include a high school diploma, LPN license or MA certification, and 3–5 years of nursing experience with

Qualifications

Minimum High School diploma or equivalent required.
LPN licensed or MA Certification by the state in which employed.
3-5 years’ experience nursing and utilizing an EMR system.

Responsibilities

Be a valued member of the clinical team in providing patient-centered care.
Review and flag all scheduled patient charts using an electronic alert to indicate an area that the provider needs to address during the patient’s upcoming visit.
Identifies clinical opportunities where follow-up care is needed.
Order diagnostic testing as needed based on the algorithm/process as predetermined by nursing management and provider.
Manage variable from the algorithm that include (variables are dynamic): Fall Risk Assessment, Smoking Cessation, BMI, Mammogram, Bone Density, Pneumovax, Fecal Occult, Diabetic measures.
Communicate with providers and other healthcare staff as needed to assist with other tasks in the office.
Performs other duties as assigned.
