Aspiranet, a leading California-based nonprofit, seeks a compassionate Clinician for our Behavioral Health Division in Camarillo. The Clinical Case Manager provides services to children, youth and families in the TBS/IHBS program, coordinating intakes, screenings, and treatment plans toward behavioral goals.

The role is on-site in Camarillo, CA, full-time and exempt, with a salary in Southern California and a $1,500 sign-on/retention bonus. Compliance with HIPAA and agency standards is required.

Qualifications

• Intern Registration per BBS or equivalent regulatory registrations as listed.

Responsibilities

• The Clinical Case Manager provides case management services to children, youth and families in the TBS/IHBS program.
• Coordinate intakes, screenings, 30-day review meetings, trainings, and documentation toward behavioral goals.
• Develop and implement Treatment Plans with the Treatment Team.
• Attend meetings related to child/youth or family needs and Treatment Plans.
• Maintain contact with community professionals and facilitate communication and coordination of services.
• Complete quality and timely strength-based treatment plans and reports.
• Maintain client confidentiality per HIPAA and agency standards.

Position Summary

TheClinical Case Manager(CCM) provides case management services directly to children, youth and families in our TBS/IHBS program. TheCCMcoordinates, conducts and documents intakes, screenings,30 day reviewmeetings, related program trainings, case documentation, reports, and interventions toward the child's behavioral goals.

Key Responsibilities

Your Skills and Duties

• Conduct intakes for Medi-Cal eligible youth
• Captureidentifiedbehavioral goals with deliverables
• Conduct timely screenings.
• Provide appropriate referrals and develop safety plans when indicated by assessment and or screenings.
• Work collaboratively to engage the youth's TreatmentTeam in the identification of strengths and needs which inform the development and implementation oftreatmentplans.
• Facilitate the development of the TBS/IHBSTreatmentPlan.
• Attend meetings related to child/youth or family’s needs andTreatmentPlan.
• Maintain contact with community professionals involved with child/youth and family to facilitate communication and coordination of services.
• Work in collaboration with child/youth, family, family team members, community members and other collaborative partners to facilitate successful completion of service plan and to support permanency and lifelong connections.
• Develop a professional supportive relationship with children/youth, families, referring parties, collaborative partners and community.
• Complete quality and timely strength basedtreatmentplans, reports, and other case documentation per Agency, funder and regulatory standards.
• Provide case contact and support (training, resources, advocacy, and interventions) to children, youth and their families as needed to meet service plan objectives.
• Document all contacts per Agency standards.
• Secure required documents to maintain files per Agency, funder and regulatory standards.
• Utilize program database and related reports to meet program standards.
• Respond to and report all incidents, complaints and grievances per program, Agency and regulatory guidelines.
• Adhere to all program, Agency and regulatory requirements.
• When requested by supervisor, carry reduced caseload in order to support projects, initiatives, child and/or family trainings and /or specific program functions.
• Conduct and participate in Agency sponsored trainings and special events.
• Maintain appropriate professional boundaries with staff, clients and community resources.
• Participate in scheduled case review, individual and group supervision, and staff / program meetings.
• Maintain client and case confidentiality as per HIPAA, state and Agency standards.
• Secure and maintain adherence to program consents.
• Accurately complete administrative paperwork (time sheets, mileage, time off requests, expense reimbursements, weekly schedule, etc.) on time.
• Support and model Aspiranet’s values, represent the Agency in a professional manner, and abide by the Agency’s Professional Code of Ethics.
• Demonstrate sensitivity and responsiveness to cultural differences.
• Perform additional duties as assigned by Core Program Director/Manager/Supervisor.

Qualifications

• Master’s Degree in a related field required.
• Intern Registration with the Board of Behavioral Sciences as an ACSW, AMFT, APCC or Registration as a Psychological Assistant with the Board of Health and Welfare
• Meets all compliance requirements of federal, state, and county regulations by periodic sanction checks.
• Meets all state required conditions of employment as set forth by Community Care Licensing for a Licensed Community Care facility; i.e. Fingerprint and Child Abuse Index clearance, TB/Health Physical, valid CDL, and clean driving record.

Work Environment & Schedule

This position is based in Camarillo, CA, with an expectation of on-site presence, including oversight of evening and weekend coverage schedules as needed.
