Job summary

Lovelace Health System in Albuquerque, NM seeks a day shift, PRN Clinical Case Manager RN to join our Acute Care team. The role focuses on coordinating care, discharge planning, and family advocacy to ensure optimal patient outcomes.

Requirements include an RN license (NM or compact within 90 days), BLS, ACLS/PALS per department, and a BSN/MSW preferred plus CCM/ACM credentials. Sign-on bonus up to $10k and relocation assistance may be available.

Qualifications

• BS or BA in Social Work or ADN or Diploma in Nursing.
• Social Worker: Medical Social Worker, Licensed Clinical Social Worker preferred.
• Must have NM license for Social Worker and BLS certification.
• RN: NM Nursing license or NM compact license within 90 days.
• BLS must be obtained within 14 days; ACLS/PALS may be required by department.

Responsibilities

• Assess social, psychosocial, cultural, environmental and financial situations.
• Advocate for patient/family rights and involve them in care decisions.
• Coordinate acute, rehabilitative and long-term discharge processes for optimal care.

Get to Know Your Team:

• Lovelace Westside Hospital provides care to Albuquerque’s Westside and Rio Rancho, including a 24-hour emergency department and a wide range of inpatient and outpatient surgical capabilities such as Ear Nose and Throat (ENT), bariatric and orthopedic service lines.

Responsibilities

• The Clinical Case Manager assesses the social, psychosocial, cultural, environmental and financial situations concerning the patient and family.
• He/she is a patient/family advocate, promoting rights and dignity and striving to involve the patient/family in aspects of care as appropriate.
• The Clinical Case Manager also facilitates the acute, rehabilitative, and long-term discharge processes to provide for an optimal age-specific continuum of care for the populations listed below.

Qualifications

Job Requirements:

• BS or BA in Social Work or Associate\'s Degree in Nursing or Diploma in Nursing.
• Social Worker:
• Medical Social Worker, Licensed Clinical Social Worker preferred.
• Must have current license as Social Worker from New Mexico Board of Social Work Examiners, and basic life support certification.
• Registered Nurse:
• Valid NM Nursing license or NM compact license (within 90 days of hire)
• BLS certification must be obtained within 14 days of hire or transfer into the role and prior to providing direct patient care.
• HIRING DEPARTMENT MAY ALSO REQUIRE ACLS and or PALS.

Preferred Job Requirements:

• Master’s Degree in Social Work or Bachelor’s Degree in Nursing
• Two years of medical social worker or RN Case Manager experience
• CCM or ACM
