Job summary

Aviva Health in Roseburg, OR is seeking a Clinical Care Specialist to join the Same Day Clinic team. You will provide efficient, patient-centered clinical support for acute illnesses and injuries, collaborating with physicians, nurses, and medical assistants to ensure timely evaluation, treatment, testing, and follow-up care.

The role requires licensure/certification aligned with your credentials, strong clinical judgment, and the ability to work in a fast-paced ambulatory setting.

Qualifications

• Current CCMA certification in good standing.
• Minimum 5 years of relevant clinical experience as CCMA.

Responsibilities

Position Purpose:

The primary function of the Clinical Care Specialist is to serve as an integral member of the Same Day Clinic team, providing efficient, compassionate, safe, and patient-centered clinical support to patients presenting with acute illnesses, injuries, and urgent medical concerns.

The Clinical Care Specialist works collaboratively with physicians, advanced practice providers, registered nurses, medical assistants, and other members of the healthcare team to support timely patient evaluation, treatment, monitoring, procedures, diagnostic testing, and follow-up care.

The Clinical Care Specialist performs duties consistent with the employee's professional licensure or certification, education, training, demonstrated competency, and applicable scope of practice. Depending on the employee's credentials and scope of practice, responsibilities may include patient assessments, clinical procedures, medication administration, diagnostic testing, emergency response, care coordination, patient education, and assisting providers with clinical procedures.

All clinical activities are performed in accordance with applicable federal and state regulations, Oregon regulatory and scope-of-practice standards, Aviva Health policies and procedures, established clinical protocols, and direct guidance from onsite licensed medical providers.

Essential Duties and Responsibilities:

• Performs patient intake and obtains appropriate medical history, chief complaint, allergies, current medications, vital signs, and other pertinent clinical information within the employee's scope of practice.
• Performs full or focused patient assessments dependent upon the employee's licensure or certification and promptly communicates abnormal findings or changes in patient condition to the on-site medical provider.
• Assists with the triage of walk-in and scheduled patients using established clinic protocols.
• Identifies patients requiring immediate evaluation or escalation of care and promptly communicates concerns to the medical provider.
• Supports efficient patient flow by preparing patients, rooms, equipment, and supplies for examinations and treatments.
• Assists providers with examinations, treatments, procedures, and other clinical interventions.
• Monitors patients before, during, and after treatments and procedures and reports changes in condition promptly.
• Provides patient education regarding medications, treatment plans, procedures, wound care, follow-up instructions, and other aspects of care as directed by the medical provider.
• Performs clinical procedures in accordance with the employee’s licensure or certification, scope of practice, training, demonstrated competency, and organizational policies.
• Recognizes signs and symptoms of potentially emergent or life-threatening conditions and immediately notifies the appropriate provider.
• Responds to medical emergencies occurring within the clinic and provides assistance appropriate to the employee's licensure, level of training and scope of practice.
• Assists with stabilization and preparation of patients for transfer to a higher level of care.
• Performs CPR and other emergency interventions consistent with current certification, training, and scope of practice.
• Administers medications and treatments only when permitted by the employee's licensure or certification, scope of practice, provider orders, organizational policy, and applicable law.
• Verifies appropriate patient identification and follows established medication administration and safety procedures.
• Accurately and timely documents patient assessments, vital signs, procedures, treatments, testing, medications, patient education, and other clinical information in the electronic medical record.
• Communicates patient needs, abnormal findings, changes in condition, and other pertinent information to providers and members of the healthcare team.
• Responds to patient telephone calls and electronic communications within established protocols and the employee's scope of practice.
• Assists with referrals, diagnostic testing, prescriptions, follow-up appointments, and other care coordination activities as directed by providers.
• Assists patients with understanding discharge instructions and follow-up recommendations.

QUALIFICATIONS

Candidates must meet one of the following qualification pathways:

Certified Clinical Medical Assistant (CCMA)

• Current nationally recognized Certified Clinical Medical Assistant (CCMA) certification in good standing.
• Minimum of five (5) years of relevant clinical experience as a CCMA.
• Current Basic Life Support (BLS) certification.
• Demonstrated competency in ambulatory clinical procedures and patient care.

Emergency Medical Technician (EMT)

• Current Oregon EMT certification in good standing.
• Minimum of five (5) years of relevant clinical experience as an EMT.
• Current Basic Life Support (BLS) certification.
• Demonstrated competency in patient assessment, emergency response, and clinical procedures appropriate to the employee's certification and training.

Licensed Practical Nurse (LPN)

• Current Oregon Licensed Practical Nurse (LPN) license in good standing.
• Minimum of two (2) years of relevant clinical experience as an LPN.
• Current Basic Life Support (BLS) certification.
• Demonstrated competency in focused patient assessment, medication administration, clinical procedures, and other nursing responsibilities appropriate to the employee's scope of practice.

Paramedic

• Current Oregon Paramedic license in good standing.
• Minimum of two (2) years of relevant clinical experience as a Paramedic.
• Current Basic Life Support (BLS) certification.
• Demonstrated competency in emergency assessment, stabilization, medication administration, and clinical procedures appropriate to the employee's scope of practice.

Registered Nurse (RN)

• Current Oregon Registered Nurse (RN) license in good standing.
• Minimum of two (2) years of relevant clinical experience as an RN.
• Current Basic Life Support (BLS) certification.
• Demonstrated competency in patient assessment, medication administration, clinical procedures, and other nursing responsibilities appropriate to the employee's scope of practice.

Additional Qualifications:

• Experience in primary care, urgent care, emergency medicine, same-day access, ambulatory care, or another comparable clinical environment preferred.
• Strong clinical judgment and ability to recognize changes in patient condition.
• Ability to communicate clinical information accurately and promptly to providers and other members of the healthcare team.
• Ability to work effectively in a fast-paced environment with multiple competing priorities.
• Excellent organizational, communication, interpersonal, and critical-thinking skills.
• Demonstrated ability to work independently while appropriately seeking assistance or escalation when needed.
• Experience with electronic medical records preferred.
• Proficient computer skills, including Microsoft Office applications and electronic health record systems.
• Ability to learn and maintain competency in clinic-specific workflows, equipment, procedures, and protocols.
• Must maintain all required licensures, certifications, registrations, and competencies throughout employment.
