Job summary

CenterWell Senior Primary Care in Morrow, GA is seeking a Clinical Care RN to support transitions of care, monitor Medicare Advantage quality metrics, and educate patients. The role emphasizes data-driven improvement and collaboration with interdisciplinary teams to enhance outcomes.

The position requires an RN license and 3+ years of relevant experience, with in-person clinic duties five days a week and periodic market travel. Bilingual abilities are a plus.

Qualifications

• ADN or BSN required.
• Active, unrestricted RN license.
• 3+ years in clinical nursing with care transitions/quality improvement.
• Experience with EMR and data analytics tools.
• Basic Life Support certification.

Responsibilities

• Analyze data in Athena EMR and DataHub to identify care gaps affecting Stars/HEDIS.
• Coordinate post-discharge follow-ups to prevent readmissions and ED visits.
• Outreach via phone, telehealth, and in-clinic visits to close care opportunities.
• Collaborate with providers, care team, and QA staff to implement interventions.
• Document encounters accurately per CMS standards.
• Participate in center huddles and quality improvement initiatives.
