VitalCaring is seeking a Clinical Care Manager (RN) to lead coordination across the full patient journey in home health. You will guide clinical decisions, manage physician orders, and ensure compliant, high-quality care in patients\' homes.

The role requires an active RN license, BSN preferred, and strong skills in care coordination and leadership. This full-time, salaried position offers autonomy with support, a focus on patient outcomes, and meaningful impact.

Qualifications

• Active RN license in state of practice.
• Graduate of an accredited nursing program (BSN preferred).
• Home Health experience strongly preferred.
• Experience in care coordination, case management, or clinical leadership preferred.
• Strong clinical judgment and ability to manage multiple priorities.
• Confident communicator with the ability to collaborate across disciplines.
• Comfortable working independently in a fast-paced, field-based environment.

Responsibilities

• Lead and coordinate care across the full patient journey.
• Coordinate and integrate patient care across clinicians, caregivers, and providers.
• Manage physician orders, documentation, and workflows within the EMR.
• Partner with Intake to support timely admissions and reduce referral delays.
• Conduct ongoing patient assessments and ensure plan-of-care alignment.
• Review clinical reports and take appropriate action.
• Lead case conferences, recertifications, and discharge planning.
• Ensure compliance with clinical, state, and federal regulations.
• Serve as a key liaison between physicians, patients, families, and internal teams.
• Support care delivery through in-home visits as needed.
