Job summary

VitalCaring is seeking a Clinical Care Manager (RN) to lead coordination of home health care in Oklahoma City. You will oversee care plans, guide clinical decisions, and ensure high-quality patient outcomes through collaboration with clinicians, caregivers, and providers.

This full-time, salaried role offers autonomy with structured processes, strong leadership support, generous PTO, and opportunities to impact patient experience. RN license and Home Health experience preferred.

Qualifications

• RN license active in the state of practice.
• BSN preferred; strong nursing education background.
• Home Health experience strongly preferred.
• Experience in care coordination, case management, or clinical leadership is valued.
• Excellent clinical judgment and ability to manage multiple priorities.

Responsibilities

• Lead and coordinate care across the full patient journey in home health.
• Manage physician orders, documentation, and workflows within the EMR.
• Partner with Intake to support timely admissions and reduce referral delays.
• Conduct ongoing patient assessments and ensure plan-of-care alignment.
• Review clinical reports and take appropriate action.
• Lead case conferences, recertifications, and discharge planning.
• Ensure compliance with clinical, state, and federal regulations.
• Serve as liaison between physicians, patients, families, and internal teams.
• Support care delivery through in-home visits as needed.

What You'll Do

Lead and coordinate care across the full patient journey:

• Coordinate and integrate patient care across clinicians, caregivers, and providers
• Manage physician orders, documentation, and workflows within the EMR
• Partner with Intake to support timely admissions and reduce referral delays
• Conduct ongoing patient assessments and ensure plan-of-care alignment
• Review clinical reports (occurrences, infection control, on-call) and take appropriate action
• Lead case conferences, recertifications, and discharge planning
• Ensure compliance with clinical, state, and federal regulations
• Serve as a key liaison between physicians, patients, families, and internal teams
• Support care delivery through in-home visits as needed

What We're Looking For

We're seeking RNs who understand the pace, accountability, and impact of home health care:

• Active RN license in state of practice
• Graduate of an accredited nursing program (BSN preferred)
• Home Health experience strongly preferred
• Experience in care coordination, case management, or clinical leadership preferred
• Strong clinical judgment and ability to manage multiple priorities
• Confident communicator with the ability to collaborate across disciplines
• Comfortable working independently in a fast-paced, field-based environment
