VitalCaring Group in Port Charlotte, FL is seeking a Clinical Care Manager (RN) to lead the coordination of home health care across the full patient journey, align plans of care, and guide clinical decisions to ensure high-quality outcomes.

You will manage physician orders and EMR workflows, collaborate with intake for timely admissions, and lead case conferences, discharge planning, and compliance with state and federal regulations. This on-site role offers full-time salary and dedicated PTO.

Qualifications

• This role requires an active RN license in the state of practice.
• BSN degree preferred.
• Home Health experience preferred.
• Experience in care coordination or clinical leadership.
• Strong clinical judgment and ability to manage multiple priorities.
• Confident communicator and collaborator across disciplines.
• Comfortable working independently in a fast-paced field-based environment.

Responsibilities

• Coordinate and integrate patient care across clinicians, caregivers, and providers.
• Manage physician orders, documentation, and workflows within the EMR.
• Partner with Intake to support timely admissions and reduce referral delays.
• Conduct ongoing patient assessments and ensure plan-of-care alignment.
• Review clinical reports and take appropriate action.

• Ensure compliance with clinical, state, and federal regulations.
• Serve as a key liaison between physicians, patients, families, and internal teams.
• Support care delivery through in-home visits as needed.

What You’ll Do

Lead and coordinate care across the full patient journey:

• Lead case conferences, recertifications, and discharge planning
• Ensure compliance with clinical, state, and federal regulations
• Serve as a key liaison between physicians, patients, families, and internal teams
• Support care delivery through in-home visits as needed

What We’re Looking For

We’re seeking RNs who understand the pace, accountability, and impact of home health care:

• Active RN license in state of practice
• Graduate of an accredited nursing program (BSN preferred)
• Home Health experience strongly preferred
• Experience in care coordination, case management, or clinical leadership preferred
• Strong clinical judgment and ability to manage multiple priorities
• Confident communicator with the ability to collaborate across disciplines
• Comfortable working independently in a fast-paced, field-based environment
