Community Behavioral Health in Philadelphia seeks a Clinical Care Manager to deliver integrated, whole-person care management for individuals with complex behavioral needs. You will coordinate care across systems, monitor utilization management activities, and promote collaboration among providers, members, and support networks to improve health outcomes.

The ideal candidate has strong clinical skills, Medicaid experience, and a commitment to health equity for all Philadelphians.

Qualifications

• Master’s degree in Social Work or related clinical field required.
• Licensed Social Worker (LSW), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), or Licensed Clinical Social Worker (LCSW) preferred; full licensure must be obtained or eligible.
• Minimum of 3 years of direct clinical experience providing behavioral health services.
• Experience with Medicaid population and commitment to health equity.

Responsibilities

• Review and coordinate utilization management (UM) activities per criteria and regulations.
• Engage in care coordination with treatment planning, meetings, and crisis consultations.
• Monitor progress toward goals, provider performance, and documentation quality.
• Consult with Physician and Clinical Supervisors; participate in case conferences and supervision.
• Maintain compliant documentation and respond to correspondence within set timelines.
• Advocate for members to receive care in least restrictive settings and uphold HIPAA standards.
