Job summary

Qualifications

• Active clinical license required from RN, LPN, PT, PTA, OT, COTA, ST or CSW.
• Bachelor's degree preferred to support clinical admissions work.
• 3–5 years of clinical experience in acute or post-acute care or senior living.
• Experience with EMR/CRM systems and care coordination is advantageous.

Responsibilities

• Achieve occupancy and census goals.
• Review referrals and clinical documentation to determine admission eligibility.
• Conduct clinical assessments and coordinate admissions with internal teams.
• Ensure smooth transitions between hospitals, skilled nursing, and other levels of care.
• Triage hospitalized residents and coordinate appropriate placement or return to care.
• Build and maintain relationships with hospital case managers, discharge planners, physicians, social workers, and other referral sources.
• Track referrals, admissions, and professional contacts within CRM/EHR systems.
• Collaborate with admissions, clinical, and operations teams on referral and census management.
• Support outreach events, marketing initiatives, and community engagement efforts.
• Assist with training and guidance of Admissions Coordinators.
• Serve as backup to the Admissions Director and provide coverage as needed.
• Participate in on-call, weekend, and emergency response responsibilities.
