Job summary

HCR Home Care in Rochester, NY is seeking a licensed Physical Therapist to provide in-home therapy services to patients per physician orders. The role requires evaluating and treating patients, developing plans of care, and coordinating with physicians.

You will document clinical notes, educate patients and caregivers, participate in case conferences, and ensure safety and equipment maintenance. This is a full-time position with a competitive annual salary range of $80,000 to $95,000.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current, valid Physical Therapy license issued by NYS Department of Education.
• One year of practical experience as a licensed physical therapist.

Responsibilities

• Evaluate patient with muscle, nerve, joint and functional tests.
• Prepare initial evaluation and periodic re-evaluations; plan of care.
• Recommend plan of care modifications to physician.
• Document clinical notes in the medical record.
• Treat patients to improve functional independence.
• Educate patients and caregivers in use of prosthetic, orthotic and DME.
• Attend staff meetings and case conferences.
• Participate in Plan of Treatment development with physician.
• Ensure safety practices and equipment maintenance.
• Other duties as assigned.

Role and Responsibilities

A qualified professional, who directs, supervises, evaluates and provides physical therapy services to patients in the home , following physician’s orders.

Essential Functions

• Evaluate patient using diagnostic muscle, nerve, joint , and functional ability tests and measures.
• Prepare an assessment of the patient’s condition at the time of the initial evaluation and perform periodic re-evaluations, develop a written problem list and plan of care including types and frequencies of treatment.
• Recommend modification of plan of care to physician, as needed.
• Document clinical notes on the date of the visit into medical record and keep all records and summaries up - to - date .
• Treat patients to address impairments and functional limitations , with a goal of reach in g maximum functional independence.
• Instruct and/or recommend patients and their caregivers in the use of prosthetic, orthotic , and durable medical equipment (canes, walkers, wheelchairs, etc.) .
• Include and educate the pat i e n t and caregiver ( s ) regarding treatment plan and goals.
• Attend physical therapy staff meetings and in-service presentations.
• Participate in case conferences.
• Participate in the development and periodic revision of the physician’s Plan of Treatment.
• Assure appropriate safety practices and maintenance of equipment.
• Other duties as assigned.

Education Requirements

• Graduate of an accredited P hysical T herapy program.

The Physical Therapist is primarily in a non-office setting and may be exposed to outdoor conditions.

The working conditions are classified as medium work:
