Baptist Health in LaGrange, KY is seeking a Clinic Specialty Pharmacist to join our Central Pharmacy. The role supports ambulatory clinic teams, provides clinical medication counseling, and manages complex therapies for specialty patients.

You will supervise Specialty Pharmacy Care Coordinators, collaborate with physicians, nurses, MAs and providers, and participate in program development to promote optimal patient outcomes.

Qualifications

• Bachelor’s or Doctorate in Pharmacy required.
• Kentucky Board of Pharmacy license required.
• 1–3 years related experience or ASHP accredited residency preferred.
• Ambulatory clinical pharmacy experience preferred.
• Specialty disease state experience preferred.

Responsibilities

• Enroll specialty patients into medication management programs and assist with recruitment in clinic.
• Provide initial clinical medication counseling and regular reassessments via in person, telephonic, or video encounters.
• Perform medication therapy management to optimize patient regimens and outcomes.
• Refer patients to financial counseling for copay assistance when needed.
• Collaborate with physicians and clinicians to ensure exemplary specialty pharmacy service.
• Maintain clinic services, policies, and pharmacotherapy workflows; respond to clinical escalations.

This position is based in a specialty medical clinic setting.
