Overview

Licensed Clinic Director - Care Coordination Program

Therapy + Care Coordination: The Best of Both Worlds! We’re seeking a licensed therapist to support our innovative care coordination program. This role allows you to practice at the top of your license, expand into care coordination duties, and enjoy flexible scheduling, excellent benefits, and the chance to make a significant impact on seniors' lives.

Responsibilities

  • Oversee patient care in senior living facilities, including Independent Living, Assisted Living, Memory Care, and Skilled Nursing.
  • Ensure services (Therapy and Wellness) comply with regulations and align with company policies.
  • Provide assessments, treatment planning, and therapeutic interventions.
  • Contribute to program development, quality improvement, and clinical initiatives.

Minimum Qualifications

  • Degree from an accredited OT, PT, COTA, PTA, or SLP program.
  • Active state licensure in OT, PT, COTA, PTA, or SLP.
  • Excellent communication skills.
  • Results-oriented with strong critical thinking.
  • Tech-savvy with quick learning ability.
  • Minimum of one year management experience preferred.
