SAILS Washington is hiring a Client Intake & Transitions Manager for Supported Living Services to connect potential clients with our programs and coordinate smooth transitions into supported living environments.

The role focuses on residential referrals, meet-and-greets, and collaboration with client care teams, with a strong emphasis on advocacy and person-centered care. Travel across Washington State is required.

Qualifications

  • High School Diploma and ability to obtain background clearance.
  • 2 years' experience working with clients with disabilities.
  • 3 years supervisory experience.
  • Must pass Washington State background check.
  • Valid driver's license and reliable vehicle; travel throughout Washington State.

Responsibilities

  • Manage all Supported Living Services residential referrals and coordinate timely placements.
  • Submit referrals within 10 days of receiving them.
  • Track referrals on the referral tracker and maintain accurate data.
  • Facilitate meet-and-greet appointments between prospective clients and care teams.
  • Participate in client transition meetings to ensure smooth onboarding into SLS programs.
  • Work with the Client Care Team to ensure person-centered placements.
  • Collaborate with Area Directors to fill program vacancies and maintain 90% census.
  • Collaborate with Representative Payees to establish client financial support prior to enrollment and housing.
  • Establish relationships with nonprofit housing partners for IDD clients.
  • Advocate for clients’ needs and goals for proper placements.
  • Support program growth by identifying new enrollment opportunities and improving service delivery.
  • Maintain documentation and reporting on referrals, transitions, and client needs.
  • Ensure compliance with WA State SLS regulations and standards.

Summary:

The Client Intake & Transitions Manager for Supported Living Services (SLS) connects prospective clients with our programs and ensures smooth transitions into supported living environments. This role focuses on residential referrals, meet-and-greets, coordination with client care teams, and supporting client financial and personal needs prior to program enrollment. This position advocates for clients, ensures person-centered care, and supports program growth by building strong relationships with clients, families, and community partners.

Essential Duties and Responsibilities:
  • Manage all Supported Living Services residential referrals and coordinate timely placements.
  • Submit referrals within 10 days of receiving them.
  • Track referrals on the referral tracker, submit them timely to the Residential Director for review, and maintain accurate data.
  • Facilitate meet-and-greet appointments between prospective clients and care teams.
  • Participate in client transition meetings to ensure smooth onboarding into SLS programs.
  • Work with the Client Care Team to ensure potential placements are person-centered.
  • Work with Area Directors to fill program vacancies and maintain 90% census at all times.
  • Collaborate with Representative Payees to establish client financial support prior to enrollment.
  • Collaborate with Representative Payees to help secure housing for new clients.
  • Establish and maintain relationships with nonprofit organizations that support housing for IDD clients.
  • Advocate for clients’ needs, preferences, and goals to ensure person-centered, appropriate placements.
  • Support program growth by identifying opportunities for new client enrollment and improving service delivery.
  • Maintain accurate documentation and reporting on referrals, transitions, and client-specific needs.
  • Ensure compliance with all Washington State SLS regulations and organizational standards.
  • Other duties as assigned.
Required Education and Experience:
  • High School Diploma
  • 2 years' experience working with clients with disabilities
  • 3 years supervisory experience
  • Must pass a Washington State background check
  • Valid driver's license and reliable personal vehicle (mileage reimbursed)
  • Must be able to travel throughout Washington State for meet-and-greets, site visits, and community outreach
Preferred Education and Experience:
  • Bachelor's Degree in Human Services or a related field
  • 3+ years working with people with disabilities
  • 5+ years managing teams
  • Experience in Supported Living Services, developmental disabilities services, case management, or a related field
  • Knowledge of person-centered planning and SLS best practices
Licensing / Training Requirements:
  • Certified in Crisis Prevention Institute, Right Response, or similar training
  • CPR/First Aid certified
  • Pass background check screening upon hire and every two years thereafter, or as requested
  • Maintain active NAR, CPR/First Aid, Nurse Delegation, and other required certifications
  • Complete a minimum of 12 hours of continuing education annually

Position Type and Expected Hours of Work:

Full-time, Salary Exempt. Overtime expected. Typical hours: Monday through Friday, 8:30 AM-5:30 PM, with frequent after-hours attention required.

Working Conditions:

Combination of office-based work and in-person client visits, including residential settings. Travel within Washington State is required. Out-of-state travel is typically not necessary.
