Position Summary

The Day Program Client Care Coordinator supports adults with Intellectual Disabilities in Pioneer Center Day Programs. The Coordinator serves as the primary advocate for each client, ensuring compliance, quality of life, and progress toward goals set through the Person-Centered Planning Process. The role collects data to evaluate program effectiveness, efficiency, and stakeholder satisfaction.

Essential Functions
  • Carries a caseload within DHS community day guidelines.
  • Establishes contact with guardians and/or family members and facilitates communication between families and agency staff.
  • Completes monthly service reviews, implementation strategies, ICAPs, CIRs, progress notes/billing, and consents, entering all information via the Agency’s EHR system.
  • Possesses knowledge of entitlements and serves as a resource to clients, families, and other agencies.
  • Prepares all relevant paperwork and presents at the Human Rights Committee (HRC) as needed.
  • Ensures compliance with auditors such as BQM, BALC, CARF, HCBS (DHS), etc.
  • Generates and collects program data, writing client goals, training staff on goals, monitoring, and collecting data.
  • Informs the direct supervisor of all conditions that may be detrimental to the client’s well‑being.
  • Conducts all phases of client service in compliance with confidentiality laws and HIPAA.
  • Leads a program based on HCBS Settings Rules requirements for the Day Program.
  • Possesses excellent verbal and written communication skills.
  • Maintains regular, reliable, predictable attendance.
  • Maintains continuing education as required per DHS after QIDP certification is obtained.
  • Performs all other duties as assigned by the direct supervisor.

Skills & Abilities
  • Education: Bachelor's Degree (four-year college or technical school) in Human Services or related field such as sociology, special education, psychology, or criminal justice.
  • Experience: 1+ years working directly with individuals with a developmental disability.
  • Computer Skills: Microsoft Office (Word, Excel, PowerPoint, Outlook), Microsoft Teams, and SmartCare.
  • Certifications & Licenses: QIDP Certification or eligibility to become a QIDP following Department of Human Services standards.
  • Other Requirements:
    • Must have and maintain a valid driver's license.
    • Must obtain 12 CEUs annually to maintain QIDP certification.
    • Ability to use phone, fax, copier, and file as required.
    • Must meet agency driver safety requirement: personal vehicle insurance limits of $50,000 bodily injury per person, $100,000 bodily injury per accident, $50,000 property damage.
Physical Demands

Requires frequent standing, walking, sitting, manual tasks such as lifting, carrying, reaching, bending, grasping, and speaking. Tasks may involve occasional lifting of 10–20 lbs and repetitive activity varying from 33% to 66% of the time.
