Qualifications

Responsibilities

  • Provide and coordinate clinically competent care to patients with End Stage Kidney Disease.
  • Perform pre-, during-, and post-hemodialysis patient assessments.
  • Collaborate with patients and families to assess learning needs.
  • Direct daily provision of safe and effective dialysis therapy.
  • Administer prescribed medications and document justification.
  • Respond to emergencies and initiate appropriate interventions.
  • Ensure proper laboratory collection and processing.
  • Update care plans through collaboration with the interdisciplinary team.
  • Educate patients on transplant and treatment modality options.
  • Provide oversight to Patient Care Technicians when designated as Team Leader.

Job Summary:
Provide and coordinate clinically competent care to an assigned group of patients with End Stage Kidney Disease (ESKD). This role includes assessment, planning, intervention, and evaluation of patient needs, along with leadership and teaching responsibilities within the dialysis care team. Works under established nursing standards, policies, and applicable state and federal regulations. Key Responsibilities:
  • Perform pre-, during-, and post-hemodialysis patient assessments and document in medical records
  • Collaborate with patients and families to assess learning needs and provide education
  • Direct daily provision of safe and effective dialysis therapy in collaboration with care staff
  • Administer prescribed medications and document justification and effectiveness
  • Respond to emergencies and initiate appropriate interventions (including CPR)
  • Serve as a resource for the healthcare team and participate in staff training/orientation
  • Ensure proper laboratory collection, processing, and shipping procedures
  • Update care plans through collaboration with the interdisciplinary team
  • Educate patients on transplant and treatment modality options
  • Provide oversight to Patient Care Technicians/LPNs/LVNs/RNs when designated as Team Leader
Required Experience:
  • Graduate of an accredited School of Nursing
  • Current state licensure as a Registered Nurse
  • Current CPR/BLS certification
  • Minimum 1 year of nephrology nursing experience in the last 2 years
  • Chronic hemodialysis experience required
Nice-to-Have Experience:
  • Prior leadership or charge nurse experience in a dialysis setting
  • Additional certifications in nephrology nursing
Required Skills:
  • Chronic hemodialysis nursing care
  • Patient assessment and care planning
  • Medication administration and documentation
Preferred Skills:
  • Staff training and mentorship
  • Knowledge of transplant and treatment modalities
  • Laboratory procedure compliance
