Qualifications

• High School Diploma or GED required.
• Six months’ volunteer/health educator experience.
• Ability to complete CDSMP leader training.
• Strong verbal and written communication skills.
• Bilingual English/Spanish preferred at ALTA score 10.

Responsibilities

• Facilitate chronic disease self-management classes according to the curriculum outline and established methods.
• Recruit participants for the classes.
• Actively engage participants, using knowledge of cultural differences to create a comfortable environment for sharing and questions.
• Collect and complete participant data from classes and submit weekly records to the Program Coordinator II.
• Receive feedback from the Service Care Coordinator regarding CDSMP program requirements and modify presentation delivery accordingly.
• Consult with and provide feedback to the Service Care Coordinator and Program Coordinator II regarding the classes and participant feedback.
• Perform other duties as assigned.
