The Health Promotion and Chronic Disease Prevention (HPCDP) Branch in the Prevention Services Division (PSD) reviews, plans, designs, implements, and evaluates public health interventions for chronic diseases and related risk factors to improve health-related quality of life for all Coloradans. Particular emphasis is placed on evidence-based practices, data-driven and evidence-informed decision-making, capacity building, partnerships, and the integration of program efforts to improve efficiency and effectiveness. The statewide chronic disease programs within the HPCDP Branch include Heart Disease and Stroke Prevention, Comprehensive Cancer Control, Colorectal Cancer Control, Diabetes Prevention and Management, Alzheimer’s and Related Dementias, Healthy Eating and Active Living, the Cancer, Cardiovascular and Pulmonary Disease Grants Program, and the Tobacco Education, Prevention and Cessation Grants Program.

The Chronic Disease Integration Unit oversees a portfolio that encompasses both the Cancer, Cardiovascular, and Pulmonary Disease (CCPD) Grant Program and the Chronic Disease LPHA Grant Program. The CCPD Grant Program was created in 2005 by voters' passage of Amendment 35, which funds competitive grants to provide a cohesive approach to the prevention, early detection, and treatment of cancer, cardiovascular disease, and chronic pulmonary disease in Colorado (C.R.S. 25-20.5-302). The unit’s Chronic Disease LPHA Grant Program is funded through both the A35 (C.R.S. 25-20.5-201) and the Proposition EE tobacco program.
This position directs and leads two grant programs, the CCPD Grant Program and Chronic Disease LPHA Grant Program, with a combined annual budget of approximately $11 million, while providing formal unit supervision and direction to four FTE staff responsible for program implementation. This includes ensuring statutory and regulatory compliance, advancing strategic departmental and programmatic goals by planning, managing, and refining all aspects of the grant programs, including efforts across the grant lifecycle, committee oversight, and coordination of resources with state and community partners. The position is responsible for running the monthly CCPD review committee meetings. In collaboration with the operations unit, the position implements the annual scope of work and budget negotiation process between the program and grantees and oversees and directs the RFA solicitation, review, and award process each funding cycle. Additionally, the position meets regularly with the executive leadership of the CCPD review committee (Chair and Vice Chair) to craft the monthly meeting agenda and discuss and review the program's management and implementation.
This position leads a strategic planning process to improve alignment between current approaches, programs, and goals and the grant program's awards. The position monitors strategic factors and other critical considerations affecting awards and the program's direction on an ongoing basis. The position also represents the department and division to partners in other State and local agencies on the public health approach to chronic disease prevention and management, and on how these efforts are affected by health policies and changes in the public health landscape.

Experience Only:
Eight (8) years of relevant experience in public health program management, strategic grant administration (including competitive RFA development and fiscal governance), personnel supervision, and evidence-based chronic disease prevention or health promotion initiatives.
Or
Education and Experience:
A combination of related education with a degree in Public Health, Health Care Administration, Public Administration, Health Policy, Behavioral/Social Sciences, Nursing, or a related field, and/or relevant experience in public health program management, strategic grant administration (including competitive RFA development and fiscal governance), personnel supervision, and evidence-based chronic disease prevention or health promotion initiatives equal to eight (8) years.
