Axil Health is seeking a Care Navigator to support patients with chronic conditions across our North Carolina locations. You will reach out to assigned patients on a structured schedule, review remote monitoring dashboards, and document interactions using efficient tools.

Qualifications

• Certified Medical Assistants (CMA, CCMA, RMA, NCMA, or equivalent national certification).
• Licensed Practical Nurses (LPN) with an active NC license.

Responsibilities

• Reach out to assigned patients on a structured schedule, some weekly, some monthly, based on their condition and risk level.
• Review remote monitoring dashboards and proactively contact patients when data is trending in the wrong direction.
• Document patient interactions using tools (less typing, more connecting).
• Escalate clinical concerns to an on-team pharmacist using clear protocols.
• Assist to enroll newly eligible patients into the program.
