Job summary

Optima Medical in Arizona is seeking a Care Coordinator for an in-office, EMR-based coordination role. You will support patients living with chronic conditions by developing care plans and coordinating care with the healthcare team to achieve health goals.

This position leverages clinical knowledge from a Medical Assistant background, with EMR experience (eClinicalWorks preferred) and strong communication skills, in a bilingual English/Spanish environment. A HS diploma or CMA is required.

Qualifications

  • At least 1 year of clinical work experience as a Medical Assistant.
  • At least 1 year of EMR experience; eClinicalWorks preferred.
  • High level of professionalism.
  • Excellent written and verbal communication skills.
  • Strong customer service skills.
  • Medical Assistant Course Completion Diploma or CMA designation required.
  • Bilingual (English/Spanish) is a plus.
  • Must have HS diploma or GED equivalent.

Responsibilities

  • Develop personalized care plans with patients, caregivers, and healthcare providers.
  • Coordinate and facilitate communication among physicians, nurses, and other team members.
  • Monitor adherence to treatment plans and medications, and progress toward health goals.
  • Provide education and support on disease management, lifestyle changes, and preventive care.
  • Document patient encounters in EHR and track interventions.
  • Collaborate with social services to address social determinants of health.
