Assistance Plus is seeking a Children’s Case Manager to provide professional case management to eligible clients in the Wilton, Maine area. You will advocate for clients, coordinate services, and work within an interdisciplinary team to meet complex needs.

The role requires 18+, at least 1 year of relevant experience with children who have Serious Emotional Disturbance, and a Bachelor’s or Master’s degree in a related field. Travel and flexible hours are expected.

Qualifikationen

• At least 1 year of life/work experience with children with Serious Emotional Disturbance (SED).
• Must be 18 years of age or older.
• Bachelor’s degree from an accredited four-year institution with specialization in psychology, mental health or related field; or Bachelor’s in an unrelated field with 1+ year relevant human services experience; or Master’s degree in a related field.

General Description of Responsibilities:The scope of practice for this position is to provide professional case management service to eligible clients. The case manager is an advocate for clients when securing services and is responsible for coordinating all services deemed necessary to meet the needs of clients served. The Children's Case Manager (CCM) will be sensitive toward others and enjoy working with children and adolescents and work effectively within an interdisciplinary team.This position will be required to possess the ability and desire to work flexible hours to meet client demand.
