Job summary

Assistance-Plus in Fairfield, ME is seeking a Children’s Case Manager to provide professional case management for eligible clients and to advocate for services within a collaborative team. You will coordinate all necessary supports for children and adolescents, traveling within the service area and maintaining flexible hours to meet client needs.

The role requires at least a bachelor’s degree with related fields, and experience with serious emotional disturbance is preferred.

Qualifications

• Minimum one year relevant life/work experience with children with a Serious Emotional Disturbance.
• Must be 18 years of age or older.
• Intermediate level computer skills are required.

Responsibilities

• Provide professional case management services to eligible clients.
• Advocate for clients to secure services and coordinate all necessary services.
• Work with children and adolescents in an interdisciplinary team.
• Be flexible with hours to meet client demand and travel within the designated area.
