Terros Health is hiring a Children's Case Manager at our 27th Avenue Health Center in Phoenix, AZ.

Duties Include
  • Conducts the engagement process with families to identify natural/formal supports as outlined in the Arizona Model, Child and Family Team (CFT) process, and Regional Behavioral Health Authority (RBHA).
  • Facilitates CFT/Service Plan meetings according to the Arizona Model and RBHA guidelines in order to create individualized service plans as identified by the CFT members.
  • Ensures families understand the behavioral health process, the role of CFT Facilitator, and ensure family participation that links them to natural or community-based supports.
  • Coordinates and schedules CFT/Service Plan meetings with natural, formal supports, and system stakeholders.
  • Submits appropriate referrals to clinical outpatient services within agency and other behavioral health providers identified by the CFT.
  • Updates and maintains electronic medical records on each case which include but not limited to; Annual Assessment Update, RBHA demographics, Child and adolescent service intensity instrument (CASII), and CFT/Service Plan.
  • Completes strength assessments and crisis/safety plans with families according to RBHA and the Arizona Department of Health Services (ADHS) guidelines.
  • Develops positive supportive relationships with consumers, family, stakeholders, and other Terros employees.
  • Complete Intake/Core Assessment as needed, according to RBHA and ADHS guidelines.

Qualifications

Qualifications:

  • GED/High school diploma with a minimum of 3 years of behavioral health (BH) experience working with children, or
  • Associate degree in the BH field, or Bachelor’s degree in BH field, or Bachelor’s degree in non-behavioral health field with 1 year minimum of BH experience working with children, or
  • Master’s degree or higher in behavioral health field.
  • Must have a valid Arizona Fingerprint Clearance card (Level 1) or apply for an Arizona Fingerprint Clearance card within 7 working days of employment.
  • Must have or obtain a valid Arizona Driver License, be 21 years of age with a minimum 4 years driving experience. Must meet requirements of Terros driving policy
  • Must pass a TB Test.
