Job summary

Georgia Hospice Care's Hands of Hope program seeks a dedicated Child Life Specialist to provide home-based psychosocial support to children with life-limiting illnesses and their families. The role emphasizes compassionate care aligned with physician orders and a collaborative care team.

Ideal candidates hold a Bachelor's in a relevant field with CCLS certification and have at least two years of practice in healthcare settings.

Qualifications

  • Bachelor's degree in relevant field and CCLS certification from the Child Life Council.
  • Current CCLS certification.
  • 2 years of Child Life Specialist experience in a healthcare setting.
  • Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 25 lbs and bear the weight of an average adult effectively.

Responsibilities

  • Assess patients to identify psychosocial, financial and environmental needs as evidenced by documentation and care planning.
  • Make the initial Child Life Specialist evaluation visit and re-evaluate needs during follow-up visits.
  • Communicate significant findings to the Supervisor, physician and care team.
  • Report unsafe conditions and follow-up on outcomes by day’s end.
  • Implement the plan for patient safety using patient, family and community resources.
  • Contribute to Plan of Care development and discharge planning for quality continuity.
  • Verify Plan of Care prior to each visit and provide care per physician orders and standards.
  • Initiate and revise Plan of Care in response to issues identified.
  • Write physician orders to cover additional visits and changes to the plan of care.
  • Incorporate patient care goals into documentation and care delivery.
  • Perform skilled services/interventions per standards and physician certification.
  • Counsel and involve patient and family in following the Plan of Care.
  • Maintain continuity of high-quality patient care and coordinate with clinical staff.
  • Participate in patient care conferences and document ongoing communication.

Essential Functions
  • Assesses patients to identify the psychosocial, financial and environmental needs of patients as evidenced by documentation, clinical records, case conferences, team report, call-in logs, and on-sit e evaluation.
  • Makes the initial Child Life Specialist evaluation visit and re-evaluates the patient’s Child Life Specialist needs during each following visit.
  • Communicates significant findings, problems, and changes in condition or environment to the Supervisor, the physician and/or other personnel involved with patient care.
  • Reports unsafe conditions and the outcome of each visit to the appropriate Supervisor as appropriate by the end of the day.
  • Implements the plan for patient safety, using the patient, family, and community resources.
  • Participates in implementation and development of the Plan of Care to ensure quality and continuity of care and proper discharge planning.
  • Verifies the Plan of Care prior to each visit and provides care according to physician’s orders, assessment data, and established standards and guidelines.
  • Initiates and revises the Plan of Care in response to identified patient care issues.
  • Writes physician orders to cover additional visits and changes to the plan of care, per agency policy.
  • Incorporates patient care goals established in the plan of care, as evidenced by documentation in clinical note.
  • Performs appropriate skilled services/interventions in accordance with accepted standards of practice and certified by the patient’s physician.
  • Counsels and instructs and includes the patient and family in following the Plan of Care and meeting Child Life Specialist and related needs.
  • Assures the continuity of quality patientcare delivered.
  • Maintains communication with clinical staff providing updates and changes in schedules, patient data or needs, physician orders or special instructions.
  • Participates in patient care conferences offering clinical suggestions.
  • Assures clinical notes indicate continuing communication and coordination of services with physician, other staff and disciplines.

  • EDUCATION: Bachelors degree in relevant field and CCLS certification from the Child Life Council.
  • LICENSURE: Current CCLS certification
