Qualifications

• Master’s degree in Social Work, Psychology, Marriage and Family or other human service field.
• Must maintain or be working towards independent licensure; LICSW/LP/LPCC/LMFT preferred.
• Minimum one year or 2,000 hours of supervised experience with mental health–challenged children/families.
• Bilingual English/Spanish preferred to support diverse families.

Responsibilities

• Case load of 17–18 children/youth and their families, with standard client service expectations.
• Coordinate mental health and related services to improve child functioning at home and in the community.
• Complete documentation including IFCSPs, Functional Behavioral Assessments, Progress Notes.
• Provide diagnostic assessments as needed or assigned.

Washburn Center's case managers work collaboratively with families and across multi disciplines internally, to develop a care plan and ensure access to needed services, including mental, social, educational, health, vocational and recreational assistance. The Case Manager II is actively present in the community and home life of the client. We encourage independent working for all our Case Managers to allow for the best client care and services.

At Washburn Center for Children, we are committed to provide culturally based, individualized targeted Children's Mental Health Case Management to children/students and families in their home, schools and community based on each family's needs.

Responsibilities:

• As a Case Manager, you will be managing a caseload of 17-18 children or youth and their families with all client service expectations that accompanies this caseload.


• You will coordinate mental health and other health related services. The purpose of this involvement in the child and family's life is to improve or maintain child/students emotional and behavioral functioning at home and in the community, to help the family access needed resources for the child including obtaining or maintaining third-party insurance or Medical Assistance coverage, and to reduce the risk of out-of-home placement, risk of maltreatment, or the need for protective services.


• You will be expected to complete timely documentation and planning including Individual Family Community Support Plans, Functional Behavioral Assessments, Progress Notes and case management authorizations.


• You will complete Diagnostic Assessments as needed or assigned

Day to day activities:

Treatment - Case Managers will provide services to educate and support families to become empowered to better self-advocate and navigate the various systems with more confidence and skill, as well as the following services:

• Assessment for your clients you will complete a functional assessment of the medical, social, educational, and other services and resources the child will need to address their mental health needs.


• Service Plan Development. You will be developing an Individual Family Community Support Plan (IFCSP) based on the functional assessment and diagnosis.


• Dependent on each individual case you will Refer, Coordinate and Monitor Services

**All clinicians are to ensure Minnesota and HIPAA data privacy requirements are followed.**

• Master’s degree in Social Work, Psychology, Marriage and Family or other human service field, required


• Must maintain or be working towards independent licensure; licensure as LICSW, LP, LPCC, or LMFT strongly preferred


• If candidate has a Master’s in Social Work, valid licensure as an LGSW is required


• A minimum of one year or 2,000 hours of supervised experience working with mental health compromised children and families
  


• Flexibility, resourcefulness, and demonstrated cultural responsiveness when working with diverse populations, specifically those impacted by systemic inequality


• Must have vehicle in operating condition, a valid driver’s license, and car insurance is required
  


• Fluency in both English and Spanish is strongly preferred
