Listed is the base hiring salary range offered for this position. Actual salaries may vary depending on factors, including but not limited to skills and experience. The salary range listed is just one component of the total rewards/compensation package offered to candidates.
Min = $91,359.84
Mid = $118,767.80
Max = $146,175.75

Summary

Functions as the Chest Pain Program Coordinator for Onvida Health. The position is responsible for coordinating the comprehensive specialized care for our patients in collaboration with the healthcare team. This includes providing clinical quality and performance improvement expertise and support to enhance safe, effective, efficient, timely, equitable, patient-centered care for cardiovascular patients.

Responsibilities

Program Management Responsibilities: Collaborates with the medical director to develop and maintain a chest pain program that includes pathways, protocols, policies and procedures, and educational needs for the organization utilizing a multidisciplinary approach. Participates in patient care as when available to ensure protocols are followed and to identify opportunities for improvement. Partners with medical staff, nursing, case management, and service line directors (radiology, pharmacy, etc.) to assure patients are managed appropriately throughout the continuum of care. Serves as a liaison between chest pain program services and other Onvida Health departments, fostering positive relationships among patients, family, departments, providers, and administration.

Administrative Responsibilities: Leads team through survey preparation and serves as point of contact for survey. Works with medical director to schedule surveys. Maintains program documentation as appropriate.

Quality / Performance Improvement Responsibilities: Collaborates with Quality and the multidisciplinary team to ensure accurate measurement and analysis of registry, quality, and benchmarking data in evaluation of the program. Monitors trends in data to identify trends and potential quality and patient safety issues and facilitates appropriate interventions to improve care. Provides communication feedback to all participants of the chest pain program, including EMS, which includes opportunities for improvement and recognition of performance excellence.

Education Responsibilities: Develops and provides chest pain related education (orientation and continuing) for providers, nurses, EMS, clinical caregivers, and the community in collaboration with the medical director. Utilizes data to identify education and training opportunities for workforce and the community. Stays current on all guidelines and best practice from AHA, GWTG, DNV, and other sources as appropriate.

Committee Responsibilities: Co-chairs a multidisciplinary team with the medical director. Coordinates and conducts routine meetings. Participates in other relevant committees and councils as appropriate, including sharing program performance.

Skills and Abilities: Exercises good judgement, demonstrates effective critical thinking skills, and professionalism. Flexible, able to prioritize and address multiple responsibilities concurrently. Works effectively under stress and frequently changing job requirements/situations
