Job summary

Forest Park Nursing & Rehabilitation in Dallas, TX is seeking Licensed Vocational Nurses (LVN) and Registered Nurses (RN) to join our compassionate team. You will provide high-quality resident care, adhere to guidelines, and collaborate with physicians and staff to ensure optimal outcomes.

Qualifications

Responsibilities

• Follow charting and incident reporting procedures.
• Adhere to narcotic counting, pharmacy orders, and restraint reduction processes.
• Document changes in patient conditions including nutrition, infections, skin issues, weight loss, or other changes.
• Utilize the Quality Life Manual for patient placement and monitoring in the appropriate programs.
• Document and notify departments of patient room changes, LOA, new admits, or discharges.
• Make rounds with physicians when necessary.

Licensed Vocational Nurse (LVN) will provide outstanding care to our patients and ensure that patient’s needs are met in accordance with standards of practice, physician orders, center policies as well as state, federal and local guidelines.

RESPONSIBILITIES

• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Document and notify applicable departments of any patient's room changes, LOA, new admits, or discharges.
• Makes rounds with physicians when necessary.
