Job summary

Plainfield Health Care Center is seeking a Nurse Supervisor (RN/LVN) to ensure care delivery aligns with mission and regulations, and to coordinate nursing care during assigned shifts. You will mentor staff and collaborate with physicians to maintain high-quality patient outcomes.

The role requires Indiana licensure, 2+ years SNF experience, and BLS/CPR certification; the successful candidate will supervise on-shift nursing personnel and participate in staff development and compliance activities.

Qualifications

• Requires LVN or RN license in Indiana.
• Minimum 2 years nursing experience in a Skilled Nursing Facility preferred.
• Current BLS/CPR certification.

Responsibilities

• Ensure delivery of high-quality clinical outcomes and resident/family satisfaction on shift.
• Provide mentorship and education to nursing staff; coordinate with physicians and other professionals.
• Manage nursing personnel to limit overtime and agency use.
• Assess work performance against unit standards and goals.
• Lead staff meetings and provide shift updates to DON.
